To attend or participate in any of the programs as a rider, or auditor, you will need to sign and upload the below release forms for 3 Pillars to keep on file prior to the event. Minors must have forms completed by a guardian. Your reservation is fully secured once registration, forms, and payment are all received.

Here's what you do: 

1. Register and pay: Clinic Registration

2. Download and sign the necessary release forms below as a Rider or Auditor.

3. Once signed, scan and upload your forms to the  Dropbox folder below.

All registration forms must be signed and received before October 10, 2019.
